Health & Wellness
At Baker, the health and wellness of our students is our primary concern. It is reassuring to our entire community to have our school nurse on-site every day in case a child becomes ill, needs medication, or requires medical attention if injured.
In addition, our team of teachers and administrators collaborate with our nurse to ensure that Baker’s space is as safe as possible for children with severe allergies.
Our School nurse, Anne Riggs, can be reached at 847-425-5816 during the school day or at firstname.lastname@example.org.
Ms. Riggs is responsible for managing the daily healthcare needs of our students. In addition, she provides health education to faculty and students, maintains student health records, and manages state mandated health compliance requirements. She screens our students’ vision and hearing and arranges for all faculty to be CPR certified.
If your child has any special health needs, please contact Ms. Riggs prior to the start of the school year to discuss medical care and accommodations that will be required during the school year.
Medical Forms & Health Records
Baker Demonstration School manages medical and health forms in the Magnus Health portal. You can log into Magnus through your FACTS Family Portal log-in. If your child’s doctor needs a copy of the forms, they are linked below.
Health requirements by grade:
Current Physical Exam Form: PK, Kindergarten, 6th Grade, All New Students
Current Dental Exam Form: Kindergarten, 2nd, and 6th Grade
Current Eye Exam Form: Kindergarten and All Students New to an Illinois School (excluding preschool)
Definition of a “current” physical, dental and eye exam form:
Physical Exam Form: Exam dated August 24, 2021 or after
Dental Exam Form: Exam dated November 16, 2021 or after
Eye Exam Form: Exam dated August 24, 2021 or after
Type of health professional the exam is to be performed by:
Physical Exam Form: Health Care Provider (MD, DO, NP)
Dental Exam Form: Dentist
Eye Exam Form: Optometrist or Ophthalmologist (the exam cannot be done by a pediatrician nor can a “screening” be accepted)
If your child is transferring from another school:
In addition to the above listed requirements, Baker asks that the student’s health records are sent from the school last attended.

COVID-19 & Vaccination
Updated COVID-19 guidance for the 2022-23 academic year will be available in the Back to School communication mid-August. Baker follows gudiance from CDC, IDPH and ISBE and receives consultation from a Health Advisory Team which includes three local physicians.
Families should send proof of vaccination to email@example.com.
If you need to report that your child or someone in your household has tested positive for COVID-19 or your child has had close contact with an individual infected with COVID-19, please notify the Baker health line at firstname.lastname@example.org.
