The School Nurse cares for the overall health of the school community. She manages the CPR training of faculty, works with faculty on health education for all students, and cares for the daily healthcare needs of children in the school. She is responsible for all student health records and requests from the state for mandated school health compliance. If your child has any special health needs, please contact school nurse Anne Riggs at 847-425- 5816 or ariggs@bakerdemschool.org prior to the start of the school year to discuss medical care and accommodations that will be required during the school year. Health forms are available to download on the Baker Demonstration School website.

Ms. Shonk graduated with her bachelor’s degree in Psychology from De Paul University. She spent the earlier part of her career in quality of life outcomes research for patients facing acute and chronic illness at Evanston North Shore Hospital. Carrie then attended Loyola University Chicago’s School of Social Work where she graduated with a Master’s Degree with honors after internships working in a homeless shelter and a Junior High School. Carrie then worked at a kindergarten through eighth grade therapeutic day school. In 2008, Carrie joined Chicago Public Schools where she supported students at Hawthorne Scholastic Academy for eight years. Carrie happily joined Baker as the school social worker in 2015. Carrie has conducted workshops in the Chicago and North Shore area for parents around special education topics, most recently in May 2016 at the Erickson Institute through Chicago’s Neighborhood Parent Network. Carrie is glad to be part of Baker, a school which is academically challenging and honors child development.

1) If your child has a severe allergy to food, bees or other allergens that requires an Epi-Pen or Auvi-Q, please fill out the following form with your doctor: Allergy and Anaphylaxis Emergency Plan (PDF)
