My name is Anne Riggs and I am the new school nurse at Baker. My hours are from 9:30 to 3:00 every day during the school year. ALL MEDICAL FORMS SHOULD BE FILLED OUT AND SENT IN BEFORE THE FIRST DAY OF THE SCHOOL YEAR.

For students new to Baker:
1) You will need a doctor’s physical and up-to-date immunizations: Certificate of Child Health Examination (PDF).
Parents please fill out and sign the Medical History portion of this form yourself.

For new pre-kindergarten students:
1) You will need a doctor’s physical and up-to-date immunizations: Certificate of Child Health Examination (PDF)

For second grade students:

All Baker Students must have the Annual Health Services Survey (PDF) submitted to the Health Office by the first day of school.

Other Medical Forms

1) If your child has a severe allergy to food, bees or other allergens that requires an Epi-Pen, please fill out the following form with your doctor: Anaphylaxis Emergency Health Care Plan (PDF)
2) If your child has asthma please fill out the Asthma Action Plan (PDF) form with your doctor.
3) In order for the nurse to administer over-the-counter and/or prescription medications during school hours, please fill out and have your doctor sign the Medication Authorization form (PDF). (Please also read the attached Medication Authorization Letter (PDF).
